To receive services at West Caldwell Health Council, you must be a registered patient. Payment for services is based on a sliding-fee scale according to family income. They accept most health insurance plans, including Medicaid and Medicare. To qualify for a sliding scale fee discount, please bring the following information with you to register at their clinics: Proof of Income, for every person living in the house over the age of 18. This may include: a paycheck stub showing year-to-date income; a copy of your signed tax form reporting your income, a bank statement if you receive direct deposit and/or a copy of any social security benefits you may be receiving Proof of Residence, for example, a bill addressed to yourself sent by mail. Evidence of health or dental insurance coverage, if any. A Picture I.D. card.
