A-Mo Salina Health Center  provides: medical dental lab radiology public health wic nutrition contract care pharmacy with mail order behavioral health optometry community health service and mammography Hours: Monday, Tuesday, Wednesday, Thursday, Friday 7 AM - 5 PM Must complete application and create Chart Fees: Accepts Medicaid, Medicare, Private Pay Insurances and offers sliding scale fees. Eligibility: Must be member of a federally recognized Tribe. There are exceptions to this requirement. Contact agency for details. Documents required: To open a chart and begin services must bring Social Security card Certificate of Degree of Indian Blood (CDIB) Photo ID or Letter of descendancy  issued by a federally recognized Tribe Current insurance information (Medicaid, Medicare, SoonerCare or private insurance)
