Proof of income, if no insurance. Sliding fee billing available if eligible. FOR EVERY VISITParent or Guardian must accompany children under age 18. Current insurance cards. Payment or Co-payment is expected at time of service; we will bill insurance as a courtesy. Proof of income, if no insurance. Bring/request referrals as necessary. If you need a referral, please request it at least 48 hours prior to the appointment. Bring someone to supervise children in the waiting area, do not leave them unattended.
