For patients who do not have dental coverage, visit fees will be based on family size and household income, according to a sliding fee scale. Upon check-in the client will be responsible for paying fees based on the scheduled services to be preformed during the dental appointment (i.e. procedures, exam, x-rays, etc.) based on the sliding fee scale. Please bring the following information with you to your appointment to make the registration process go faster: Proof of income (required to qualify for discounted sliding fee scale) Examples are pay stub, tax returns or a letter from employer Copy of insurance card (Medicare, Medicaid, private insurance) All medications you are taking (prescription, herbal, over the counter) Address verification (utility bill, phone bill, etc.) Services: Oral exams and x-rays Cleanings Extractions Fillings Sealants Root canals Crowns Dentures/Partials Custom mouthguards for sports Additional Services: Free books for children Oral Health Education' Prayer Support Discount Prescription Medication Program Spiritual Counseling Free Vitamins for Children Children's Oral Health Program (Tooth Fairy Program)

Many of our Dental Clinics are free and/or government supported. Many are sliding scale dental care for low income. Please call the dental clinics or visit the websites of the dentists to cofirm that these clinics are free or low cost to the needy. We do our best to provide a helpful low cost and free dental services. Always confirm directly with the dental clinis to confirm that our information is up to date.
