Broward county health department north regional health center. Clinical preventive and restorative dental services for children between 2.5 to 20 years old from families whose income is below 200% the Federal OMB poverty level; program's goal is to improve the dental health of the youth of Broward County. Fees charged:  Medicaid Self-Pay/Full Self-Pay/Sliding scale (family's income must be 200% below Federal Poverty Level) Intake procedure:  Appointment proof of income social security card tax statement parental consent and residence address required Eligibility requirements:  Must be 2.5-20 years old for Medicaid, family income below 200% of Federal Poverty Level Languages spoken or available:  English Spanish
