Hours of operation:   Monday, Wednesday and Thursday 8am-5pm; closed Noon - 1pm; closed on Friday Fees charged:  No fee, Medicaid, Family's income must be below 200% of Poverty Level Intake procedure:  Appointment proof of income and residence social security card Eligibility requirements:  Adults who are defined as eligible under federal poverty guidelines. Must have referral from MHS Primary Care PRogram Languages spoken or available:  English Area served:  Broward County Preventive and limited routine dental care for eligible adults that are referred to the clinic through the Memorial Health Care System Primary Care Program. The Primary Care Physician makes a referral based on pre-established citeria. Services provided: Dental and Healtcare.
