From toothaches, cavities, and cleanings, to restoring your smile by fixing or replacing teeth. Dental Services:  Cleanings  Exams  X-rays  Dental sealants  Restorative treatment  Extractions Community Health Partnership of Illinois accepts many payment options including Medicaid, Medicare, and many insurance plans. For those without insurance they offer convenient, low-cost easy payment options on a sliding scale. Hours:  Tuesday, 10:00 am - 8:00 pm.  Wednesday and Thursday, 12:00 pm - 8:00 pm.
