Sam Hider Community Clinic  provides medical, dental, lab, radiology, public health, wic, nutrition, contract care, pharmacy with mail order, behavioral health, optometry, community health service and mammography. Hours: Monday, Tuesday, Wednesday, Thursday, Friday 7 AM - 5 PM Must complete application and create Chart Fees: Accepts Medicaid Medicare Private Pay Insurances and offers sliding scale fees. Eligibility: Must be member of a federally recognized Tribe. There are exceptions to this requirement. Contact agency for details. Documents required: To open a chart and begin services must bring Social Security card Certificate of Degree of Indian Blood (CDIB) Photo ID or Letter of descendancy  issued by a federally recognized Tribe Current insurance information (Medicaid, Medicare, SoonerCare or private insurance)
