Health Care Network of South Florida - UF Pediatric Dental  provides general dental services that may include: routine examinations cleanings fluoride treatments fillings and tooth extractions general anesthesia interceptive orthodontics IV sedation nitrous oxide sedation spacers specialty referrals (orthodontics) X-rays sealants fillings and extractions Must bring proof of income: Pay stubs (last four weeks of income) Last Years Income Tax Return or Schedule C if self-employed Workers Compensation (last four weeks of income) Unemployment Compensation (last four weeks of income) Disability or SSI Benefits letter Social Security Income Documents from local, state, or federal agency Letter of Support (notarized signature) Completed Employment Verification Medicaid made application or rejection letterPicture identification: Drivers license, I-94 (If foreign born), Passport Fees: Based on sliding scale fee and most insurances accepted Medicaid/MedicareNo one will be denied access to services due to inability to pay Hours: Monday-Thursday 8am-5pm, Friday 8am-12pm
